Health Assessments

Health assessments are a Medicare initiative aimed at allowing people who may be at risk of illness or who are in a particular age bracket, to have a comprehensive health assessment completed. At Pioneer Health we are happy to perform any of these health assessments for you and usually book you to see a nurse followed by your doctor to complete the assessment and make recommendations. Best of all, health assessments are bulk billed so there is no out of pocket cost to you. Have a look at the following list and see if you qualify for a health assessment.

What is a Health Assessment?  
An Annual Health Assessment is available to all patients over the age of 75 and is an in-depth look at all aspects of your health. This assessment provides a great opportunity to review your overall health and general wellbeing and discuss with your doctor any concerns you may have. A Health Assessment consists of a 45 minute appointment with a nurse and a 30 minute appointment with your GP. To book in, please call our friendly reception staff on 9842 2822 and ask for a Health Assessment.

What does it cost?
Health Assessments are bulk billed (i.e. there is no out-of-pocket cost).

What happens at a Health Assessment?  
The following assessments may be completed:
  • Blood pressure, pulse and rhythm
  • Weight and blood sugar level
  • Foot check
  • Urine check
  • Alcohol intake
  • Smoking status
  • Sleep
  • Immunisation status e.g. Flu vaccine
  • Physical function e.g. walking
  • Falls risk screen test
  • Mood and memory test
  • Assessment discussed with GP and recommendations suggested

Can be offered every 9 months for all age groups.
